If you are new to Glow Day Spa, we request that you fill out a client form for our facial or body treatments. A client form provides our certified estheticians and stylists with the necessary information to complete a treatment safely and without complication. Conditions such as pregnancy (or possibility thereof), recent surgery, allergies and other various conditions can conflict with certain treatments. It is in your best interest to complete the necessary forms to the best of your ability. All information collected on a client form is strictly confidential and is only shared with the estheticians and hair stylists who will be completing your treatment or service.
